Resident Applications

Markhaven Inc. is a provincially licensed Long Term Care Facility.  To apply you need to contact your local Community Care Access Centre (CCAC). 

For example, if the applicant resides in Burlington and is applying to come to Markhaven Inc., you must call and apply through the Burlington CCAC office.  They will send out a package for completion and assessment of suitability, and will set up a case file for the applicant.

The local CCAC office will send the information to our local York Region CCAC office and they in turn will send the information to us for review and acceptance based on our ability to accommodate the medical needs of the applicant and availability. Usually applicants are placed on our wait list and we will forward a letter of acceptance to the applicant once the review has been completed.  The process can take several weeks.

Residents with limited income who are applying for "basic" or shared accommodation can apply for government accommodation subsidy.  To apply you will need to provide the last income tax year's official "Tax Assessment".  For further information you can refer to the Ministry of Health & Long Term Care web site at: http://www.health.gov.on.ca.
